Milan has three airports, one on each side of the city — Malpensa (49 km to the north-west), Linate (8 km to the east) and Bergamo Orio al Serio (57 km to the north-east). Which one is yours changes the price and the route completely, so each has its own page with terminals, meeting points and fixed fares. Booking ahead means the driver is in the arrivals hall before you are, your flight is tracked, and the price you agree when you book is the price at the end of the ride.
When a transfer beats the train. One passenger with hand luggage will do fine on the express. But with suitcases, or after a night flight, a private car gives you what the train cannot: door to door with no change at Centrale, 60 minutes of free waiting if your flight is late, child seats and room for the cases.
